The objective decides everything else
A plan whose objective begins "students will understand" cannot be assessed, because understanding is not observable. The whole point of an objective is to name what a student will visibly do, which is why Bloom's revised taxonomy is phrased entirely in verbs. Swap "understand photosynthesis" for "explain how light intensity changes the rate of photosynthesis" and both the activity and the exit check follow almost automatically.
Timing is where plans fail in practice
Most plans allocate too much time to teacher explanation and too little to the part where students produce something. The pattern that survives contact with a real class front-loads a short retrieval task, keeps direct instruction under roughly a quarter of the lesson, gives the majority to guided and independent practice, and reserves the last few minutes for a check that tells you whether to reteach. Plans that skip the final check hand the teacher no information at all.
Alignment is the common silent failure
An objective at the analysis level assessed by a recall quiz measures nothing the objective claimed. This mismatch is easy to miss because both halves look reasonable on their own. Comparing the verb level of the objective against the verb level of the assessment catches it before the lesson runs, which is the only point at which it is cheap to fix.
