Nonces must be consumed strictly in order

Every account has a counter that increments by one with each transaction it sends, and the network will not process nonce n+1 until nonce n has been mined. A single transaction stuck at a low nonce therefore blocks every later transaction you have sent, regardless of how much gas those later ones offered. This is why a wallet showing five pending transactions usually has one problem rather than five, and why paying more for the newest one changes nothing.

Cancelling means replacing, not deleting

There is no way to withdraw a transaction from the network. What is called cancellation is sending a different transaction with the same nonce and a higher fee, so that validators prefer it and the original becomes invalid once the nonce is consumed. The usual form is a zero-value transfer to yourself, which does nothing except use up the nonce. Most clients require the replacement to bid at least ten percent more, and a replacement that fails to clear that margin is simply ignored.

A gap can also come from the other direction

If a wallet has been used on two devices or reset its local state, it can send a transaction with a nonce that is too high, leaving a hole. Nothing after the hole will ever be processed, and the transactions sitting behind it are not stuck for a fee reason, so bumping their fees does nothing at all. The fix is to send a transaction that fills the missing nonce exactly, which releases everything queued behind it at once.

Frequently Asked Questions

Why are all my transactions stuck?

Because nonces are consumed in order. One transaction stuck at a low nonce blocks every later one, so five pending transactions usually mean one problem rather than five.

Can I cancel a transaction?

Not delete it. You replace it by sending a different transaction with the same nonce and a higher fee, usually a zero-value transfer to yourself that does nothing but consume the nonce.

How much higher does the replacement fee need to be?

Most clients require at least ten percent more than the original, and a replacement below that margin is dropped rather than queued.

What is a nonce gap?

A missing nonce, usually from a wallet used on two devices or one that reset its local state. Nothing after the gap can be processed, and bumping fees on those transactions does nothing because the problem is not the fee.

What does resetting a wallet account do?

It clears the locally cached nonce so the wallet re-reads it from the chain. It does not remove anything already broadcast, so it fixes a wallet that has lost track and not a transaction the network already holds.
