A RACI is only useful if it is enforced

The responsibility assignment matrix assigns four letters to each task and role pair. Responsible is who does the work. Accountable is the single person answerable for the outcome. Consulted are those whose input is sought before the decision. Informed are those told afterward. The value comes entirely from the constraints: exactly one accountable person per task, at least one responsible person, and a deliberate separation between being consulted and being informed.

The four failures

Most RACI charts in circulation break at least one rule. Two accountable people on a task means nobody is accountable, because each can reasonably defer to the other. No responsible person means the task has an owner but no doer, which surfaces as a missed deadline nobody predicted. A role consulted on everything becomes a bottleneck and stops reading. And a chart where most cells are filled has not made any choices, it has just recorded that everyone is somewhat involved, which was already true.

Build it where decisions are contested

A RACI is most valuable for the small number of tasks where ownership is genuinely ambiguous: the handoffs between teams, the approvals, the things that fell through the gap last time. Building one for every task in a project produces a document nobody reads. This generator validates each rule as you fill the grid and flags the roles that are consulted so often they will become a bottleneck, which is the failure mode that quietly slows a project without ever being raised.

Frequently Asked Questions

What is the difference between responsible and accountable?

Responsible people do the work and there can be several. The accountable person is answerable for the result and there must be exactly one. In practice the accountable person is who gets asked why it did not happen.

Why can there only be one accountable person?

Because shared accountability is no accountability. When two people are equally answerable, each can reasonably assume the other is handling it, and the failure mode is that neither does. This is the rule most worth enforcing.

What is the difference between consulted and informed?

Consulted is two way and happens before the decision: their input can change the outcome. Informed is one way and happens after: they need to know but cannot change it. Confusing the two either wastes people’s time or blindsides them.

What about RASCI and other variants?

RASCI adds Supportive for people who provide resources without owning the outcome, and RACI-VS adds Verifier and Signatory for regulated approvals. They are useful in large programmes and unnecessary overhead in most projects.

How many tasks should a RACI cover?

Only the ones where ownership is genuinely unclear: handoffs, approvals and anything that went wrong last time. A chart covering every task in the plan is documentation rather than a decision tool, and it will not be read.
