Camera filenames tell nobody anything
A file called DSC_04821.JPG carries no information for a search engine, an accessibility tool or a person looking through a folder. A descriptive filename is a small ranking signal for image search and a large convenience for everyone maintaining the site. It is also the cheapest optimisation available, since renaming happens once and is never revisited, and it is the one most consistently skipped.
The characters that break on the web
Spaces become percent twenty in every URL, making the address unreadable and awkward to paste. Uppercase letters create separate URLs on case sensitive servers, so a link to Photo.jpg fails where the file is photo.jpg. Accented and non-Latin characters are percent-encoded into unreadable sequences. Underscores work and hyphens are the convention, which matters mainly for consistency. Every one of these is trivially avoidable and each causes real breakage.
Alt text is for people first
Alt text exists so that a person using a screen reader receives the information the image conveys. That purpose determines what good alt text is: it describes what matters about the image in context, not what a keyword tool suggests. A decorative image should have empty alt text so it is skipped entirely rather than announced. Stuffing keywords into alt text degrades the experience for the people it exists to serve, and gains almost nothing.
