Initials are the fallback for a missing avatar
Almost every application shows a coloured circle with one or two letters when a user has no profile picture. Producing those letters looks trivial and is the source of a recurring class of embarrassing bug: a hyphenated surname yielding three letters and overflowing the circle, a name with a particle producing a lower case v, a single word name producing one letter where the layout assumed two, and a name in a script with no case producing something unreadable at small size.
The rules that actually work
Take the first letter of the first component and the first letter of the last component, skipping particles such as van, de and bin, which are part of the surname rather than separate names. For a hyphenated component take the first letter only, or both if the design allows three. For a single component name take one letter rather than inventing a second. For scripts without case, such as Chinese or Arabic, take the first character as it is, since forcing upper case does nothing and truncating a syllable can change the meaning.
Colour should be deterministic
The background colour of a monogram avatar should be derived from the name by a hash, so the same person always gets the same colour across sessions and devices. Random colours make an interface feel unstable and remove the recognition value that is the main benefit of the avatar. A hash also spreads names evenly across the palette without any coordination.

Frequently Asked Questions
How many letters should a monogram have?
Two is the standard, taken from the first and last name components. One is correct for a single component name, and inventing a second letter from nowhere is worse than showing one. Three overflows most circular avatars at small sizes.
How are particles handled?
Van, de, der, la, bin and similar are treated as part of the surname rather than as separate components, so Ludwig van Beethoven gives LB rather than LV. This matters because the particle is not a name in its own right.
What about hyphenated names?
The first letter of the hyphenated component is used by default. Taking both letters gives three initials in total, which overflows most avatar designs, so it is offered as an option rather than as the default.
How should non-Latin names be handled?
By taking the first character as it appears. Scripts such as Chinese, Japanese, Arabic and Hebrew have no case distinction, so forcing upper case does nothing, and a single character is frequently a complete meaningful unit already.
Why should the avatar colour be deterministic?
